
In recent years, cosmetic procedures like the Brazilian Butt Lift (BBL) have gained immense popularity, particularly with the rise of social media. But not all BBLs are created equal, and the newer “liquid BBL” procedure has raised significant concerns within the medical community. Unlike the traditional BBL, which transfers fat from one area of the body to the buttocks, a liquid BBL relies on dermal fillers or other injectables to achieve a similar effect. While marketed as a less invasive alternative, liquid BBLs carry serious risks that every patient should be aware of. Here’s why I, as a physician, have chosen not to train or perform liquid BBLs—and why you should think twice before considering one.
What is a Liquid BBL?
A traditional BBL involves liposuction to remove fat from areas like the abdomen, thighs, or flanks, which is then processed and reinjected into the buttocks to enhance shape and volume. This autologous fat transfer method has its risks, but it is generally safer when performed by a trained, board-certified surgeon under strict guidelines.
In contrast, a liquid BBL uses synthetic fillers, most commonly hyaluronic acid-based or other types of biocompatible substances, to add volume to the buttocks. The primary appeal of liquid BBLs is that they don’t require fat harvesting, making it an option for patients who may not have sufficient fat stores. However, the fillers used in a liquid BBL can migrate, lead to uneven results, and create complications that may be difficult or impossible to reverse.
Risks Associated with Liquid BBLs
While the appeal of a non-surgical enhancement can be strong, liquid BBLs carry a number of concerning risks.

Embolism and Tissue Necrosis: Fillers injected too deeply can enter blood vessels, causing embolisms that block blood flow, leading to tissue death (necrosis) and even fatal complications.

Infection: Any injectable carries a risk of infection, but the risk can be heightened with larger volumes often used in liquid BBLs. Infection could lead to severe inflammation, abscess formation, and potentially life-threatening systemic complications.

Migration and Lumps: Unlike fat, fillers can migrate over time, leading to asymmetry and a bumpy or lumpy appearance. This can create significant aesthetic issues and can be extremely difficult to correct without additional invasive treatments.

Unknown Long-Term Effects: Because liquid BBLs are relatively new, there is limited research on the long-term effects of synthetic fillers in larger volumes in the buttocks area.
A Recent Tragedy in the Industry
One recent, heartbreaking case brought to light the severe consequences associated with BBLs and liquid BBLs. A practitioner, who was not fully trained and was operating in a facility lacking essential medical safeguards, performed a BBL that tragically resulted in the patient’s death. This tragic event underscores the importance of choosing not only a trained and certified professional but also a provider who will prioritize patient safety above trends.
Why I Don’t Perform Liquid BBLs
As a medical professional, my responsibility is to weigh the potential benefits of a procedure against the risks. For me, liquid BBLs don’t meet the safety standards I uphold in my practice. Given the lack of long-term data, the immediate risks, and the prevalence of adverse outcomes, I cannot in good conscience recommend or perform liquid BBLs for my patients.
Ultimately, patient safety is paramount, and I believe it’s my duty to inform my patients of the risks associated with liquid BBLs. While traditional BBLs, when performed with care and precision, can provide satisfactory results, liquid BBLs introduce an unpredictable and, in my opinion, unacceptable level of risk.

Weigh all options carefully, and make sure you fully understand the risks before deciding on any cosmetic procedure.
